Did the Scandinavian churches ever accept the Formula of Concord?

The Church of Sweden did (in the 17th century, I think). The Church of Denmark-Norway (now the Church of Denmark and the Church of Norway) never did.

Have those churches historically struggled with the 'crypto-calvinism' the Formula was intended to remedy?

Greetings, father. Was there ever a direct papal proclamation against the validity of swedish lutheran orders as there was against those of the church of england? Forgive lack of capitalization, i'm working on dictation software.

I’m actually not sure but I don’t think so. There will have been later, with the introduction of women priests and bishops, but I haven’t heard of a Church of Sweden equivalent of Apostolica Curae.
